Tax Form W-4S - Request for Federal Income Tax Withholding from sick pay?

My boyfriend just got surgary on his foot. He gets sick pay thru his union for the time he is off. On the back of the form that his doctor had to fill out, there is a W-4S From. This is the form for "Request for Federal Income Tax Withholding from Sick Pay."

It states taht FICA and Medicare Taxes are automatically going to be deducted from his benefit checks and that the minimum FIT deduction of $20.00 is voluntary. Does he need to fill this out now, or can he pay any Federal income taxes from this time in April? Thanks.

He'd be wise to fill it out now, but if he isn't going to be off long and won't get much from the sick pay, he could just let it slide - if he does, any tax he owes on it would make his refund less when he files, or if he owes more than his refund would be, he would have to pay the extra.
